Brief summaries:
1. Respiratory muscle involvement in ALS using diaphragmatic needle EMG:
The major aim of this study is to investigate the correlation between lung function, respiratory muscle function, neurological status and electromyographic signs of diaphragmatic involvement using needle electrodes. Further we investigate if there is any difference between bulbar and limb onset ALS in respiratory muscle involvement. First results demonstrate very early diaphragmatic involvement even in the absence of any clinical signs.
2. Respiratory muscle training in ALS:
In patients with chronic obstructive lung disease and in children with Duchenne mucular dystrophy we have already shown that respiratory muscles can be trained like any other skeletal muscles. We developed an electronic device for specific inspiratory muscle training. It is based on strength and endurance training using an adjustable resistance and providing a visual feedback for the patient. In cooperation with pulmonologists we study the effect of specific inspiratory muscle training at home on inspiratory muscle strength and endurance, clinical course, intercurrent infectious diseases and quality of life in ALS patients.
